"D'oh!" Moments in games

Go time mother humpers!
Hey everybody, anyone have any funny stories to tell about how you've screwed up big time in a game. I know I have a couple.

The other day I was playing Cave Story and I had to get this item then fight a boss, when the boss fight wouldn't start I figured I did something wrong and started the whole game over. When I finally got to the same part it happened again, then I realized all I had to do was examine the item to get the boss fight. :lol
 
Getting the very end of Dark Carnival in Left 4 Dead 2 on the 360, then running from the right side to the wrong side of the stadium to get to the chopper (it goes to different sides on the PC an 360 versions) and dying.
 
For some reason on my first playthrough of Dragon Age Origins, I was in Redcliffe village trying to get all of the undead. I was wondering why the cutscene ending the mission wouldn't start. I did the mission twice, nothing worked. Then I decided to restart. Turns out, going back to the first playthrough, there was a zombie that got stuck on the dock and I never noticed it. :lol
 
Heh, those were pretty funny :)

I played through all of No More Heroes and didn't realize you could dodge by moving the joystick til the final boss, needless to say that game is now a cakewalk on bitter. :lol

Another time I was at a friend's and we were playing Army of Two. We had to do the parachuting mission through the canyon and we kept crashing til we died over and over. After 15 minutes or so I ask him if we should try tilting the controller. :lol
 
DOH! moment for me? Resident Evil Code Veronica on Dreamcast. When you found out you NEED the Fire Extinguisher at some point AGAIN in disc 2...that really pissed me off. I wish they would at least warn you.
 
In "GTA:San Andreas" I went to pull out of my garage,and somehow the GD car rolled back into the area around a staircase leading to a basement area of sorts.I couldn't back out,I couldn't even press the triangle button to exit the car!

I had to reset,which sucks,because anyone who has played any "GTA" titles know that they're slower than a 3 legged turtle when it comes to load times.Son of....!

I'll keep that in mind when I start playing that again :lol

Mai,you must have been kicking yourself something awful.I once sold a rare sword by accident in "DQ8".I was just a tad livid. :hh
